Please complete the form below to initiate your stock gift. This form helps the Shrine ensure you receive proper credit for your gift while ensuring your gift is used for the purpose you intended. Once you complete the form and click submit, you will receive an email that contains all the information you need to transfer your stock gift. Included in the email will be the Shrine brokerage name, account name, and the number and DTC number.
